dc.description.abstract | Traditionally, the shipping industry has focused on the hardware factors, such as ship’s structure and equipment systems in order to reduce marine accidents. As a result, today’s maritime systems are technologically advanced and highly reliable. However, recent marine accident statistics show that the accident rate is still high and 80% or more of all marine accidents are caused fully or in part by human error. Thus, it is quite important to decrease the possibility of human error as far as possible. In this paper, a human error analysis technique which is based on the well-established human error frameworks such as the SHELL model and Reason’s Generic Error Modeling Systems (GEMS) is proposed and towing vessel collision accidents are analyzed as a case study in order to examine the applicability of the human error analysis technique to marine accidents. Also human errors related to those towing vessel collision accidents and their underlying factors are discussed in detail. | - |
